Are you all of the following:

  • Currently pregnant or was pregnant within the last 12 months
  • Have Type 1, Type 2, Gestational Diabetes (GDM), or at high risk of GDM
  • Live in LA County
  • Earn less than 80% of the median income in Los Angeles.
